Medicare Insurance Coverage

 SCAN coverage is only accepted at my Montclair office. 

I accept SCAN insurance via ASHLink.  Please keep in mind not all SCAN plans cover therapeutic massage.  Also, your plan may have a co-payment. 

Before booking an appointment you will need to fill out the insurance intake form so I can verify eligibility. Please click on the box below to fill out the insurance intake form. You will provide the following:

• First and Last Name
• Date of Birth
• Insurance company and ID#

Co-payment and office visit limitations may apply.
